
Life Care Plans (LCP)
A report which identifies a patients medical condition and future care requirements after an injury, and quantifies the associated costs of their care over a lifetime.
The report takes all of the circumstances into account including medical costs, nursing care and costs of daily living.
Details of future causally related medical care are outlined. The frequency and cost of all care (medical and non medical) is calculated over a lifetime. This can include:
- Doctor Visits
- Nursing Care
- Medical Equipment
- Supplies
- Durable Medical Equipment
- Procedures
- Medications
- Rehabilitation
- Diagnostic Studies
- Lab Work
- Home Care & Living Arrangements
The Life Care Plan will comment on past medical treatment & outcomes, current medical status, treatment regimen, and future medical and non-medical recommendations with associated costs as related to the injury.
The goal of the Life Care Plan is to reduce complications and secondary diagnosis, maintain functioning, and optimize independence throughout the patient’s lifespan.
